OK it look like your Hard Drive is failing.

You will need to replace your Hard drive.

thanks for your reply. I have 2 hard disks. Please let me know if there is a software available to check the harddisk. Please guide.

anything else you can suggest to check ? actually i want to make sure that my hard disk is damaged, because it might be the harddisk cable issue or the mother board issue. may be the hard disk port on mother board.
