It happens only SOMETIMES, and quite rarely:
When I have booted, XP (SP2) shows ONLY the wallpaper, no icons, no "Start ", no toolbar at the bottom, nothing else.

When I start anew, all is OK.

    Why does that happen?
    How can I prevent it?

It's probably just a bad bootup....You could try 187 different things and not know for weeks if anything fixed the problem since you stated it "rarely" happens.

I suppose you are right - and may be they will be 178, but I still have some hope that someone would have something more concrete...

How about adding explorer.exe to the startup folder?

I think I'll try that... It probably would do no harm if it gets a start signal twice...

    annoyances

If you have Google desktop installed you may want to check out this
    thread on Annoyances board:

Seems everyone on that thread though, the problem happens on every boot.

I can't think why you'd get an intermittent problem unless some other application has some memory issue and it depends on where it happens to load. What happens when you boot in safe mode?

Don't know the answer, but to save time (by avoiding the reboot) call up taskmanager and start explorer from there (if you can't get a run window by winkey-r).

Look in your event viewer when it happens and see if there's something in there related to it. Start>run>eventvwr.msc
