    My daughter's lap top:
System: a lap-top Toshiba Satelite A200-FS6, OS: Windows Vista Home service pack2, cpu Intel Pentium

At turning on the lap-top, only the logo from Toshiba comes up and the MS loading bar, and then Launch repair mode.
Nothing happens except for the window :startup repair: Last successful boot time 6/2/2011 2;31: 28 AM (GMT)

    Session details
    ...............
    System disk = \Device\Harddisk0
    Windows directory =C:\windows
    AutoChk Run = 1
Number of root causes = 1

I have started in safe mode and tried "repair your computer" and this came up:

    Stop:c000021a {Fatal System error}
The windows subsystem process terminated unexpectedly with a status of 0xc0000096 (0x75a50001 0x00f7f5c4) the system has shut down.ps initially the desk top came up, but none of the icons could be activated.

Then I tried "restore ", it started to load the files, but it hangs (half an hour before I tried the next step) at Loaded: \windows\system\32\drivers\crcdisk.sys

Next I tried in the debugging mode. File system on D the volume is clean
    Checking file system on C:
One of your disks need to be checked for consistency.
Windows will now check the disk
CHKDSK is verifyng ect.(I could not keep up with the rest), but it then went to try to load. After a long while I tried the next step
Last known good configuration: desktop comes up, but icons again don't work and it stops. Can I use a copy of my computer Windows Vista Premium.

I had a similar problem when my desktop failed to boot and stopped at the end of the initial window even before Windows was to be loaded. I tied many of the things you did including checking my HD with CHKDSK and other utilities. My image backups wouldn't load and the only files I was able to recover were the data, photo and music files, etc. that were backed up into the cloud using Carbonite. I finally gave up and formatted the HD and reinstalled the original OS. Of course, I had to reinstall all of my applications and update them when necessary. I also had to update the OS using Windows Update.

After considering all of the things you tried, you have probably reached the point where further testing will prove little about why your system is failing to load. Accordingly, cleaning (formatting) the HD and reinstalling the OS may be the best option. I assume you don't have the original Vista Home disk and want to use the Windows Vista Premium disk that came with your computer.

With a clean HD, installing your Vista Premium should be no problem; however, you will find that, at some point during the installation or initial start-up, you will be requested to activate your new OS installation. I found that activation works fine the first time you use an OS installation disk; however, activation may fail when attempting to reuse the same disk and the same product key.

Microsoft is generally quite understanding when you have had a system failure or are abandoning a computer for a new one; so they will provide a new product key that will successfully activate. Generally, however, an OS installation disk is intended to be used on only one computer.

If you don't activate as soon as that page appears, you have 30 days to use the software after which some parts of the program may be disabled. In any case, you should minimize you connection time to the Internet by delaying activation until you have installed an anti-virus program to protect your computer from infections and malware. The few minutes it takes to activate is enough for the bad guys to send a virus your way so the anti-virus program is the first priority.

Then you need to use Windows Update to bring the OS up to the latest iteration. Now, you can proceed with activation and install your programs.

Thanks Bobengel, I will ask her to double check to see if she can find her installation copy.

I tried the complete factory recovery, the error code that comes up is
10-FA11-045D It shows on a google search that it may be a faulty hard drive. How can I check if that is the case since chkdsk hangs after a while.

Thanks Pete, will the checker work from a flash drive if I can not initiate windows?
Every check stalls at \windows\system\32\drivers\crcdisk.sys or hangs after what seems like a half installation

Making a bootable flash drive is not easy, as yet.

See if you can make a bootable diagnostic CD with the Toshiba utilities.

I don't hold my breath for anything spectacular when running Toshiba utilities.

Unlike other drive manufacturers it would appear that Toshiba do not offer a DOS version of their diagnostic software from which a bootable CD can be burnt - the DOS version must be used if the computer will not boot into Windows. I think you are out of luck here as I guess the laptop does not have a floppy drive.

However, checking around a bit crcdisk.sys performs a cyclic redundancy check on the drive during boot up and a failure indicates bad blocks on the disk.

My take on this is that the hard drive has failed. I hope your daughter has a current backup of her data :)

If not all may not be lost - after replacing the drive connect the old drive to a desktop computer - an adaptor will be required, and, if the drive is recognised it should be possible to copy the data off and transfer it to the new drive.

Thank you all for the replies. Unfortunately, she has not a back-up copy nor the restore cd's. My guess is that it would not be worth the $'s to replace the hard drive because she would have to buy an new OS as well. If I am wrong on this thought, please reply.

You're thinking along the right lines :) Clearly the cost of a replacement hard drive plus an OS gets you pretty close to a new laptop - with Windows 7 of course.

The laptop is ~4 years old and there may be other issues lurking in the background.

The comment I made re hitching up the old hard drive to a desktop still applies and please remember - backup, backup and backup again :)

Thanks for the confirmation of my thought.
