1.44 disks didn't exist back in the 4.0 days. Need to find a 720.

You could try a 1.44 disk and put a piece of tape over the hole without the slide cover and format it as a 720.

    OK.My questions are:
    1) Do I leave the plastic tab intact or remove it?

    2) how do I format to 720 KB?

The "tab" is a write protection thingie. Can lockit so no other data can be written to the disk. Might try Kevin's idea or ask around. Old farts like me have a few dozen of the older floppies. Heck, I've got a bunck of 5 and 1/4 stuff.
