    Open the IE browser.

From the menu bar open Tools-> Internet Options...

    On the "Connections" TAB:
Make sure that the "Never dial a connection" radio button is selected. This is the manual setting.

Also from the "Connections" TAB, single left mouse click on the "Settings..." button:
In the "Automatic configuration" area, make sure that the
"Automatically detect settings" is NOT checked.

    Close the browser.

I think you're on the right track, it's a browser function that is opening the dial up when you use it.

The file association for HTM is Internet Explorer so perhaps your IE has some corruption. Try the repair tool for it in ControlPanel > ADD/Remove Programs.

Also check in My Computer > Tools > Options > File Types > scroll to HTM and click the Advanced button and see that it isn't calling for any program other than IE to open with.

    Got me here.
