Graphic for Windows Workstation

I am looking for the graphic for the windows workstation window.

We use the Novell Client 483.SP1.

Upon boot, the Novell Client login box appears. If the user changes from the last successful boot, then a "windows workstation" window appears.

That window is the graphic I need, because you actually "login" upon filling it out, you can't print screen.....


In more detail the window looks like this:


Top of window says "Windows Workstation "

Name: (white box to be filled in)
From: (grayed out box "this is the netbios name "
Password: (white box to be filled in)

Check box with words "Change your windows password to match your Netware password after a successful login "

If you use remote control software (like WinVNC, pcAnywhere, terminal services, remote desktop, etc.), you should be able to capture the screen. Sample images...
