Forms in Word 2000

    I feel like I'm asking a really dumb question today, but I was wondering if someone could give me some quick tips on how to create a form in Word.

    I'm whipping up a questionnaire, and typically I'd do it in Access, but most of the people I'll be sending this to do not have Access, so I'm stuck doing it in Word - which I apparently know nothing about! :D

    Anyway, I want the receiver to open the template and have a form pop up which they can fill out, submit, and have it auto populate into the template. Anyone want to run down a quick step by step. Let's just do something basic, like the following:
    Name:
    Course Title:
    Date Completed:

    All help is greatly appreciated as the "help" files just don't seem to be working for me!

    regarding the auto-populate...

    I've used a Word template that initially opens a form when you first open the template. This form has the appearance of a form like you'd see in Access. You can fill in the pertinent info and click OK. The information is then transferred to the template and all fields auto-populate throughout the document that correspond with those on the form.
