IRQL_NOT_LESS_OR_EQUAL + usbohci.sys = BSoD

    Here's the situation.

    I am running a Soltek Qbic 3801A (a SFF PC). The motherboard died inexplicably in December and I RMAed the board. When I replaced it in January, BSoDs seem to occur randomly, always with IRQL_NOT_LESS_OR_EQUAL. After several times, I noticed it seemed to be associated with usbohci.sys

    The configuration between the old board and new board did not change. Even the BIOS versions were the same. To my knowledge, the CMOS settings are also identical to what I had previously.

    A friend of mine told me to open up the minidump files in a debugger to look at the culprit. It was indeed usbohci.sys

    From what I can understand, this is probably a corrupted driver soemwhere trying to access what it shouldn't (Pool corruption)?

    Some somebody here give me some pointers on how to track down the offending driver? I will post the minidump file contents here for your reference.

    To my knowledge, all my drivers are up to date. I have even uninstalled then let Windows redetect and reinstall the USB driver.

    Any help you guys can provide would be greatly appreciated. This problem is driving me up the wall!

    your symbols are still wrong, but i can see whats happening.

    Your machine is in the middle of servicing an ISR at IRQL 5. Its trying to dereference some data, and the data is invalid. We can see from the stack that this is a usb device. Without a proper stack, i can't tell for sure whats going on.

    You need to make sure your BIOS, any associated firmware, and usb port, hub and device drivers are current.

    BIOS is current.
    I will remove and reinstall all the USB devices, but how can I be sure that the reinstalled drivers are current? Windows usually does these things automatically.

    Also, what do you mean by associated firmware?

    Finally, what would I need to do to get a "proper stack" for you to look at?

    Since messing around with the drivers and such, I noticed that the BSODs were becomming more frequent, especially when the CPU is under load.

    I have no USB devices connected at the moment, so I don't know how a usb device can be causing these crashes.

    get a new dump with all your usb devices disconnected.

    The debugger tool needs access to the internet to download symbols. If you have a proxy, NAt or something that would prevent winsock based internet access, you need to tweak it a bit.

    No USB devices are plugged in! All the ports are empty! :confused:

    I don't suppose the front USB ports are counted as a device? They've always been interfaced with the motherboard though.

    Being a SFF rig, it's a huge pain in the ass to do so. I'll take a look at the earilest opportunity, probably tomorrow. Would removing the motherboard connector for the front USB ports do the trick?

    Just came to a revelation.

    When I opened my rig to remove the USB from the motherboard, I saw that instead of being connected to the USB3 header, it was connected to the USB4 header.

    This may have changed from when I had it on my old motherboard. Could that possibly be the cause of the crashes?

    I have removed the connection all together for now, fingers crossed! Gonna run a DVD for a while to see if it crashes, but somehow... the computer "feels" slightly different... a little faster perhaps.

    Ran memtest86. No problems detected.

    Anyway, the USB header seems to have solved the problem. I'll give it a few days of testing before I decide though.

    I was wondering that myself, but since we have two dumps from different sessions, crashing in the same place both times with the same error, the probability of bad data is quite high, so i wasn't bringing that up for now.

    My current guess is that onboard usb hub/controller/whatever is having intermittant failure, coming back up and renotifying with some garbage. May need to go into bios to disable it, unplugging may or may not be enough.

    Worth doing certainly because if it shows errors, you got problems.

    Conversely, software memory checkers that show clean simply mean they didn't see problems but that absolutely does not mean there weren't any.

    About the only mem testers that you can really rely on are free-standing devices that you plug the memory stick into and run from there.

    Disabling USB functions is simply not an option for me. I can stand to lose my front USB ports (if indeed they prove to be the problem), but I need at least two functional ports.

    I will run the machine for a few more days, then reconnect the front USB ports to the correct header and test it again.


    So far though, the machine is stable (Nerovision encoded fine, games/video/dvd played fine).

    I doubt it is a memory problem because at one point, I removed one stick of RAM, then the other, and the problems did not cease. Would it not be unlikely that both sticks of RAM would go bad at the same time?

    in my opinion, the probablity of it being bad ram, based on the data presented here puts bad ram in to the 'very unlikely' category. As newt says, software testing is not conclusive for negitive testing, but combine that with two dumps in the same place chewing on the same bad data, and your one stick swap test, i would say RAM is not a factor in this phenomenon.

    understood, regarding disabling usb. My goal in asking for that was divide and conquor troubleshooting to determine the vector. Sounds like you may have found it in a potentially bad front panel. Keep us posted with your testing.
