To scan a form or any kind of document as text rather than an image, you need to use OCR (Optical Recognition Software). Some sort of OCR software is usually bundled with scanners.

The problem is that even the best OCR programs make mistakes and the limited versions that ship with scanners tend to be very limited.

The more formatting in the original...such as a form...the worse the end result tends to be. The sharper and clearer the original...clean white paper, dark black print, no photocopier smudges or fingerprints, etc...the better the end result. OCR software tries to translate every mark on the paper into text and often guesses wrong.

Try scanning the form in using OCR software, save it as an rtf (rich text format) or doc (MS Word) and see if it comes out close enough to the original that it won't take hours of editing and straightening up to be usable. Sometimes it's easier just to reconstruct it in Word or the equivalent.
