Windows Vista Should I install this update?

I am a newcomer to this Forum and also new to Vista. Last month, I downloaded and installed Sp1 several times and made a mess of my computer. I ended up having to go back to the computer shop and getting a new hard drive and installed Vista Home premium 32 again.
I have not re-installed SP1 because of all the trouble it caused me. I'm now being notified that the update below is available. Would it be wise to install it do you think ? I'm now very nervous of installing any updates from Microsoft. I have not installed updates for Windows defender because I disabled it on the reccomendation of the engineer at the computer shop. I should be very grateful for any information that you can give me.
