Trojan horse Adload_r.JR (infection)

This trojan has been detected by AVG free, the file it is attached to is:
C:windows\system32\nsuD193.dll
Despite trying to heal with AVG, this virus keeps popping up everytime i open explorer? Not sure how this infection got into my lap top? i am using windows Vista.

Would appreciate any advice on how to safely remove this threat.
