It sounds as if you are trying to install using install.bat?

Simplest way is to double click ie-adds.reg and answer Yes when asked if you want to merge with registry.

Ie-spyad adds a list of undesirable websites to Internet Explorer's Restricted sites list - you can see which by going, in IE, Tools > Internet Options > Security > Restricted Sites and click on the Sites button.

Ie-spyads is not a scanner - it provides a degree of permanent protection in that it prevents you going to these sites - details here

Putting a site's URL into Restricted Sites usually does not stop you from going to the site.
Per the first paragraph in the link PeteC provided
"IE-SPYAD adds a long list of sites and domains associated with known advertisers, marketers, and crapware pushers to the Restricted sites zone of Internet Explorer. Once you merge this list of sites and domains into the Registry, the web sites for these companies will not be able to use cookies, ActiveX controls, Java applets, or scripting to compromise your privacy or your PC while you surf the Net. Nor will they be able to use your browser to push unwanted pop-ups, cookies, or auto-installing programs on your PC."
This applies to all URL's put into Restricted Sites either manually or by using IE-Spyads. There are a very few sites that will not allow entrance to the site if their cookies are blocked, but as mentioned, usually you can visit the site. Just all the listed activities are blocked.
